for Solo and ensemble entry you may

haven't.
Remember that we will print out your
entry forms and you should never try to
use an entry form that you may have
printed in this contest.
2. Now, fill out the contest summary
form and print. The contest summary
form will determine your fee amount.
3. Have a check for that amount and
send it to Brad Bouley at the address on
the home page of this site.
4. Submit a Form 1 and have a principal
sign it from uilforms.com. This document
may be brought with you to the contest.
All others must comply with the deadline
